Not a whole lot of help but...
Maybe give Greening Auto a call. They are in Nashville and may have some contacts.
Initially I thought about a build RTM/Powernation did back a while where they had custom glass cut. Since they are in Nashville (basically) thought that might help. However it turns out they may have used "Glass Guy Chicago" which may be too far out of the way for you.
