Correct Paint Code?

To all Painters.....

So this is what I was given by my painter as the paint code for my car. I need to get something "matched" don't want to call him........various reasons.

So, not being a painter, this has no meaning to me.

Question, does this appear to be all the info. you guys would need in order to match my paint?
