Any idea what happened in this case, what caused the failure? I think that's what everyone is waiting to hear. You guys have been around for a long time and make a great product, but we have a failure here and it would be nice to know what caused it. The people who already own the product I'm sure are wondering what's going on. It's great to know how your products are made, and the steps you go through to ensure quality, but in my opinion it's a mistake to not address this failure in this thread.
Improperly torqued?
Isolated incident?
Improper install?
Bad run of parts?
